New Patient Registration

coffeediaryNew patients will be asked to attend for a health check before they are accepted onto the practice list. Please attend the surgery with your medical card and bring any relevant information concerning your past health. Where a complete family is registering for the first time we ask that if possible the whole family attend together.  

All patients will also be asked to fill in a patient behaviour contract. 

The doctors welcome new patients who live within our practice area.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor. 

Medical treatment is available from the date of registration.  Please contact reception for further information.

All registration forms must be signed and a completed new patient questionnaire form must be attached, please bring it in with you on your first visit to the practice.

Please Note: If you move, you must notify the surgery of your new change of address.  Any patients moving outside these boundaries will be given 30 day notice to change to a GP nearer where you are living.
